- If your child has a food allergy, please make sure to include that information when you register online.
- If your child has been prescribed an Epi-pen, please notify the school nurse and have the Epi-pen sent to school by the 1st day of the school year.
- An emergency allergy action plan should be filled out for all students with food allergies that have been prescribed an Epi-pen. Please see the file link below.
- If you would like alternative food provided for your student from the cafeteria, a Diet Modification Request Form must be filled out. Please see the file link below. Please note that if you are choosing not to eat Pork, you do not need to fill out the form. There are non-pork food items offered every day.
