Doctor's Notes and Physicals

Bathroom Use and Water Bottles
- All students are given a planner or pass sheet at the beginning of each school year. Every student is given a certain number of bathroom passes each term.
- If a female student needs extra bathroom use because she is having her period, please instruct her to bring her planner or pass sheet to the nurse. She will be given extra bathroom passes for the week.
- If your student needs extra bathroom use in general, you need to get a note from the doctor.
- Water bottles are encouraged! They must be clear and only contain WATER.
- If your student has a sore throat or gets frequent headaches, please encourage them use a water bottle throughout the day.
- A water bottle may be taken away if a student gets in trouble with their water bottle, is sharing it with other students, or has any liquid other than water in the water bottle. Water bottle expectations are reviewed with the students at the beginning of the year.
PE Excuses
All students will be expected to dress out and participate in Physical Education.
If your student can not participate for any reason or has limitations, you must provide a doctor's note.
Please remember, if the student can not participate in PE, then they will not be allowed to participate in sports or recess either.
Physical Exam
All students wanting to participate in school sports must have a current physical form on file. Physical forms expire after 13 months from the date they were seen by the doctor. Please turn physical forms into the front desk or the school nurse. Please click on the link below for a physical form.
